At University Health Services, we understand that managing your health while keeping up with academic and work responsibilities can be challenging. This page outlines our medical excuse policy and provides guidance on how to access appointment verification through your patient portal. Our goal is to support your well-being while encouraging open communication between students, faculty, and employers.

Medical Excuse Policy

At University Health Services (UHS), our top priority is supporting student health and academic success. We are here to care for students and provide documentation when appropriate. If a student is seen at UHS for an illness or medical concern, a clinician may provide a visit verification or medical excuse at their discretion, based on the nature of the visit.

However, please note that UHS does not issue medical excuses for students who were not evaluated by our clinical staff. We encourage students to schedule routine or non-urgent appointments—such as physicals, immunizations, allergy injections, or prescription refills—at times that do not conflict with academic or work responsibilities.

 Appointment Verification

Students who need to verify a visit can access their Previous Appointment Listing through the patient portal. This can be shared with instructors or employers as needed.

To access your appointment history:

  1. Log in to the patient portal.
  2. Hover over Appointments on the left-hand menu.
  3. Select Historical Appointments to view your visit history.

If you are not web-enabled, please call us at (850) 644-4567 to speak with a representative.

Confidentiality

UHS adheres to strict confidentiality standards. We do not release information about a student’s medical or psychological condition to instructors, administrators, parents (unless the student is a minor), or employers without the student’s written consent.
