Once you have signed the above consent forms for your first visit with the BHC, you do not need to re-sign these forms for your follow-up visits.

For all BHC visits, please download and complete the PHQ-9 and GAD-7 form via the link  below prior to your scheduled telehealth appointment.  Your clinician may review this information with you during your appointment.
